You should anticipate rough terrain, potholes, and, crucially, unbridged river crossings. This is why 4x4 vehicles (often "Super Jeeps" with modified tires and high clearance) are essential for these tours, whether self-driven or with a guided tour.

Hiking Opportunities: The Highlands are a hiker's paradise. Many tours incorporate stops for short walks or offer options for longer treks, like sections of the famous Laugavegur trail (connecting Landmannalaugar to Þórsmörk).
